Records Requests
Clients have the right to request access to their mental health records. Because psychotherapy notes are written for clinical purposes and may not be therapeutically helpful when read independently, records may be redacted or a treatment summary may be provided instead, in accordance with Texas law. Clients are encouraged to discuss record requests with this therapist through any of the contact methods listed.

Filing a Complaint

Clients have the right to report any breaches of confidentiality or incidences of harm whether physical or mental to the Texas State Board of Examiners of Professional Counselors:
